Braces or Clear Aligners? An Honest Comparison

Clear aligners have been marketed hard enough that many patients now arrive having already decided, and the only question left is where to buy them. That is the wrong order. Both systems move teeth well, both are legitimate, and which one suits you depends on your bite rather than your preference. Here is the comparison without the sales pitch.

What each is good at

Fixed braces are brackets bonded to the teeth, connected by a wire. Because they are attached and cannot be removed, they work on your teeth continuously, and the wire can apply the kinds of force that rotate, upright and bodily move teeth in ways that are difficult to achieve otherwise. There is no case that braces cannot treat.

Clear aligners are a series of removable transparent trays, each slightly different, worn in sequence. They are nearly invisible, they come out for eating and brushing, and there are no wires to irritate the cheek. For mild to moderate crowding and spacing, they work very well.

Where aligners genuinely struggle

This is the part that marketing tends to skip. Aligners are noticeably less predictable for:

  • Severe crowding where teeth must be moved a long way
  • Large rotations, particularly of round teeth like premolars and canines
  • Bodily movement of a tooth through bone, rather than tipping it
  • Significant bite corrections, including deep bites, open bites and jaw discrepancies
  • Closing extraction spaces where teeth were removed to make room
  • Teeth that are short or oddly shaped, which give the tray nothing to grip

Modern aligner treatment works around some of this with bonded attachments, small tooth-coloured bumps glued to the teeth to give the tray purchase. Worth knowing before you choose aligners for invisibility: a case needing many attachments is not especially invisible.

The factor that decides most cases: discipline

Aligners have to be worn twenty to twenty-two hours a day. They come out only to eat and to brush. That is a genuine daily commitment, and it is the single most common reason aligner treatment overruns or disappoints.

Braces have no such requirement, because you cannot take them off. For a teenager, or for an adult with a demanding job and irregular meals, that is a real advantage rather than a consolation.

Be honest with yourself here. Aligners in a drawer move nothing.

Cost, time and visits

Cost. Aligners generally cost more than fixed metal braces, sometimes substantially, because each tray is custom-manufactured and the digital planning is done in advance. Ceramic or tooth-coloured braces sit somewhere in between. If cost is the deciding factor, metal braces do the most work for the least money by a wide margin.

Time. For comparable cases, treatment times are broadly similar. Aligner advertising often quotes short timelines that apply to minor front-tooth alignment, not to full correction of a bite.

Visits. Braces need adjusting every four to six weeks. Aligners need fewer chair visits, since you change trays at home, which suits people who travel.

Hygiene. Aligners win clearly. You take them out to brush and floss normally. Cleaning around brackets and wires takes real effort, and patients with braces who do not keep up with it end up with white marks around where the bracket was.

The thing both have in common

Retainers, indefinitely. Teeth drift back towards their old positions for the rest of your life. Every case, braces or aligners, ends with a retainer, and the results only last as long as you wear it. Any plan presented to you without a retainer discussion is incomplete.

How to actually decide

Get the case assessed before choosing the appliance. That means an examination, X-rays, and a clear explanation of what is being corrected: is this front teeth that look crooked, or is the bite itself wrong? Simple alignment and full orthodontic correction are different projects, and the second one narrows your options.

Ask directly: is my case one that aligners treat predictably, or one where they are a compromise? A clinic that will tell you when braces are the better answer, even though aligners cost more, is giving you the useful version of the advice.
