Do I Need a Root Canal or Just a Filling? How Dentists Decide

It’s one of the most common questions patients arrive with: “Is this just a filling, or am I in root canal territory?” The answer comes down to one thing — how deep the problem has gone.

The one-line answer

A filling repairs damage to the outer layers of the tooth (enamel and dentine). A root canal is needed when decay or injury has reached the pulp — the living nerve and blood supply inside the tooth — and it has become inflamed or infected.

Signs that point to a filling

  • Sensitivity to sweets or cold that goes away within seconds of removing the trigger
  • A visible cavity or food catching in a tooth, without much pain
  • Mild, occasional discomfort when chewing

At this stage the nerve is still healthy. Clean out the decay, place a filling, done — usually in one short visit.

Signs that point to a root canal

  • Pain that lingers for minutes after hot or cold, or arrives on its own
  • Throbbing pain that wakes you at night
  • Pain on biting or pressing the tooth
  • A tooth that has darkened compared to its neighbours
  • Swelling or a pimple-like bump on the gum near the tooth

These suggest the pulp is inflamed or already infected. A filling placed over an infected nerve doesn’t fix anything — the infection continues underneath, which is why proper diagnosis matters more than optimism.

The honest grey zone

Deep decay that sits close to the nerve is a judgement call. Sometimes a deep filling with a protective liner works and the nerve settles; sometimes the nerve was already too far gone and a root canal follows weeks later. A good dentist will tell you when you’re in this zone rather than promising certainty.
