Toothache at Night? First Aid That Helps (and What Makes It Worse)

Tooth pain has a talent for starting at 11 pm. Until you can reach a dentist, here’s what actually helps, what makes things worse, and how to tell an “endure till morning” situation from a “get help now” one.

What actually helps

A painkiller, taken correctly. An over-the-counter painkiller like paracetamol or ibuprofen (if you have no condition that rules it out — follow the label, and check with a pharmacist if unsure) is the most effective thing in the house. Take it on schedule rather than waiting for the pain to peak.

Keep your head elevated. Lying flat increases blood pressure in the tooth’s inflamed nerve — it’s why toothaches feel worse at night. Prop yourself up with an extra pillow; it genuinely helps.

Cold compress on the cheek. For swelling or injury, a cloth-wrapped ice pack against the cheek in 15-minute intervals numbs the area and slows the swelling.

Warm salt-water rinses. Half a teaspoon of salt in warm water, gently swished — soothing for gum-related pain and helps flush food from a cavity.

Clean the area. Surprisingly often, severe pain is food packed hard into a cavity or between teeth. Gentle brushing and flossing around the sore tooth sometimes brings real relief.

What makes it worse

  • Aspirin on the gum. The old remedy of holding an aspirin against the tooth burns the gum tissue — it’s acid on skin. Painkillers work from the stomach, not by contact.
  • Heat packs on the face. Heat feels nice for a moment but expands swelling and spreads infection. Cold, not hot.
  • Very hot, cold or sweet food. An inflamed nerve overreacts to everything. Lukewarm and bland until you’re seen.
  • Clove oil overdone. A drop on the tooth can dull pain briefly and is fine as a stopgap; soaking the area repeatedly irritates the tissue.

What can’t wait till morning

Most toothaches can hold with painkillers until the next day’s appointment. These cannot:

  • Facial swelling that is spreading — towards the eye or down the neck
  • Fever with the toothache
  • Difficulty swallowing or opening your mouth
  • A knocked-out permanent tooth — keep it in milk, get to a dentist within the hour

These are signs of infection spreading beyond the tooth. Don’t wait these out.

The part nobody wants to hear

Pain that vanishes on its own didn’t get better — a nerve that dies stops reporting. The infection continues silently until it returns as a swelling. A toothache severe enough to keep you up has earned an examination, pain or no pain the next morning.
